Create & Edit a Questionnaire
This guide explains how to create and customize questionnaires in Galink.
Creating a new questionnaire
Navigate to Settings > Questionnaire.
Click Create Questionnaire.
Fill in the basic information:
Title -- A clear name for your questionnaire.
Description -- Explain the purpose and scope of the questionnaire.
Questionnaire └── Section └── Question └── Option └── Evidence or Findings
Adding sections
Sections organize your questionnaire into logical groups:
Click Add Section within the questionnaire editor.
Provide a title (optional) and description (optional).
Add questions within each section.
Adding questions
For each question, configure:
Title -- The question text that vendors will see.
Description -- Additional guidance or context to help vendors answer accurately.
Answer type:
Single choice -- Vendor selects one answer from a list.
Multiple choice -- Vendor selects one or more answers from a list.
Free text -- Vendor provides a written response.
Configuring answers
For choice-based questions (single or multiple choice), define the available answer options. Each answer can be linked to:
Evidence requirement -- Specify that the vendor must upload a file or provide a comment when selecting this answer. You can also define acceptance criteria for the AI to evaluate.
Automatic finding -- Configure a finding that is automatically triggered when this answer is selected or not selected (only multichoice).
Note: An answer can link to evidence OR a finding, not both.
Editing an existing questionnaire
Navigate to Settings > Questionnaire.
Select the questionnaire you want to edit.
Modify sections, questions, or answers as needed.
Save your changes.
Important: Changes to questionnaire configuration do not impact already sent, in-progress, or closed questionnaires. Only new questionnaire sends will use the updated configuration.
Best practices
Prefer choice-based questions -- They support evidence requirements and automatic findings, enabling AI-powered analysis.
Write clear descriptions -- Help vendors understand exactly what information you need.
Define acceptance criteria for evidence -- This enables the AI Analyst Agent to automatically evaluate vendor submissions.
Organize sections logically -- Group related questions to make the questionnaire easier for vendors to complete.
